  • Please take a few moments to tell us about your item(s). We appreciate your time and effort in completing this form in as much detail as possible so the Museum staff will have adequate information to make an informed decision about your donation. (Please attach additional sheets if necessary if using a paper form).

    The information you provide will also aid the museum’s understanding of its collection and may (or may not) be used for exhibition, programming, or research purposes.

    • Please note: If the donation is accepted, transportation arrangements will be made, if necessary, and a formal agreement will be signed indicating the transfer of ownership (Deed of Gift).  In the case of a donation, the gift agreement will indicate that the donation is an unrestricted gift to America’s National Churchill Museum.
